In consideration of participating in the FAV GLAM GIRLS EXPERIENCE, MISS VIRGINIA USA®, AND MISS VIRGINIA TEEN USA® program and its related events and activities, I agree that:

1. There are inherent risks of injury.

2. I knowingly assume those risks and agree to indemnify and hold harmless, THE MISS USA®, MISS TEEN USA®, and The FAV Group, Inc. for all injuries sustained.

3. I am solely responsible for all the items I bring to the pageant. Any item lost or stolen is not the responsibility of The FAV Group, Inc.

4. I am solely responsible for any damage I may cause to my hotel room, or the pageant show venue.

I have read this release of liability and assumption of risk agreement, fully understood its terms, and sign it voluntarily.

I understand that if I am under the age of 18, I must have a parent or guardian sign this form on my behalf.
